6. Appropriate individual verbal & written post- operative instructions given to patient and escort
7. Emergency contact number given
8. A blank assessment record
9. Patient consent form for each technique assessed
10. A blank intraoperative conscious sedation record
11. A blank contemporaneous record of recovery and discharge
