Application forms

These application forms are Word documents to be printed and mailed to the address indicated on them.

Please note that in order for a place to be reserved full payment is required.

When sending a cheque with your application form please ensure that there are sufficient funds available. Unpaid cheques will attract a 10% administrative charge and result in your registration being cancelled until payment is received. Places on our courses are limited and it would be unfair to allow places to be reserved without payment.

Conditions for transfers and cancellations

To save a form to print later, right click on it and choose Save Target As.
